Mandatory extensions must be installed automatically.

Maybe we should disable caches for the install tool. Depends on how slow
this gets.
This could also indicate a bug in the cache code.

IMHO there needs to be a step (after DB) that checks for various settings
and sets sane defaults (e.g. us mbstring, IM stuff, etc).
