This would require an upgrade wizard to increase the numbers for 
existing backend_layouts (not a problem, but something which needs to be 
included).

Would there be many extensions / installations out there which are not 
compatible with belayout page module rendering?

It would be nice to have again a single rendering method, if it doesn't 
break too many things.
