[TYPO3-core] RFC #10949: Clean up sysext usersetup module code - part 2

Hi,

here is an updated patch where i changed following issues:
* use BEenableFields like Bernhard suggested
* moved CSH besides save buttons to the left to be unique with the rest, 
fullhelp now also on top of the buttons
* fixed CSS-issue with fullhelp which also fixes the view in other views.

This implicites my +1 on this patch.
