Have you tried whether emptying typo3temp/ameos_formidable helps?

Have you checked that you've included the extension's static template in
your TS template?

If you're using a customized HTML template, does switching to the
extension's default template help?

It is unlikely that using TYPO3 4.2.1 instead of 4.2.8 is the cause of
this problem. You still should urgently update to 4.2.8 so your site
won't get hacked.

(Generally, you should almost always use the latest maintenance release
of TYPO3. And you should subscribe to the TYPO3 announcements list
<http://lists.netfielders.de/cgi-bin/mailman/listinfo/typo3-announce> so
you get informed of new releases.)
