Hint 1: This is the Developer List.
Hint 2: Read the rules (there you will find...)
Hint 3: The Archive
Hint 4: Forms and CSS Styled Content don't go well together yet.
           This Info is on every list, almost every day (well, ok, it got
better in the last weeks)

Conclusion: De-Activate CSC and your form will show up OR write CSC to the
end and send your suggestions to Kasper.
