Problem:
All official TYPO3 backend-modules provide a proper docHeader view. In 
order to provide a consistent userexperience TV should have this as well.

Solution:
Attached patch adds the needed parts and moves (some) markup into the 
template-file.

Additional information:
Benny already had the concern that this makes TV incompatible with TYPO3 
4.1, but due to the fact that this is meant for the trunk only (a.k.a TV 
1.5) that's not a problem because trunk already requires 4.2+. By the 
time 1.5 is published TYPO3 4.1 will be obsolete anyways.
