i think it has to be sort of a mixture of yes/no checkboxes, 1-10 points
ratings and perhaps some free text forms as well.
also, some real life examples / references could be usefull, too.

well, i could imagine that for PR reasons the official "TYPO3 certified
extension" branding idea is not dropped at all. i think that EXT reviewing
could be a source of income for the T3 association or individuals therein.
that's OK, and i wouldn't care if they did so, as long as we have a rating
system based on community opinion and on the community plattform typo3.org.
so we could have something like a not officialy certified extension which
99% of the registered T3 users rated to be "stable & useful".

yes, something like this. also, only registered users should be allowed to
vote.
the problem with the whole idea is that the typo3.org or TER owner must
agree and help to implement some of the features...
