Hey JH,
I don't think that cached pages suffer from this problem.
However during page generation all loaded extension might get 'required',
this might take time while this is not needed.
One other thing, but releated:
I am going to do some tests today, or tomorrow about the speed
difference between require() and require_once(), I do read on the net
that require_once might not get cached well by eAccelerator for example.
I will let you all know the results...
