Here v6
As middle_name seems needed, I add it.
I place company before name, with title job in first palette.
I place first_name, middle_name and last_name in second palette. Each 
are 25 and there are linebreaks cause 3 x 25 seems too large.

Company
	- Title

Name
	- First name
	- Middle name
	- Last name
