I think it does work, but one has to set config.extTarget config.intTarget (and
if you refer to http://bugs.typo3.org/view.php?id=6626 config.fileTarget -
unfortunately 6626 hasn't made it into 4.2 yet, though it was sent to this list
on 2007-11-09 ;) ).

Setting target to _top by default doesn't seem to be the proper solution to me,
since external urls would then open in _top as well by default (though they
normally shouldn't).
And TSRef states as well, that passing '-' to target is okay...
... what do you think about these points?
