Yes, please just reply into the correct thread that you committed the
patch. If there is documentation, make sure to follow Michael's
guidelines [1] (posted here some time ago).

Yes, have a look into the changelog. _every_ change is inside there,
ordered by date, the newest on top. For bugfixing, we mostly use:
"Fixed bug #0000: short_description" - but you'll see this in the
ChangeLog anyways.

There is a NEWS.txt as well, I'll update it soon, but this one is
updated more sporadically and contains the biggest changes as a
"sum-up".

@Dmitry: about bugfixing:
Masi explained it really well already. Sometimes I create a bug report
myself and assign it to me right away to keep track of an issue and make
my work more transparent to others - especially when the changes are
bigger and I want to hear some other's opinions as well. If somebody
submits a new bug report, I mostly ask them if they can work on it
themselves, and if they supply a patch and I think it is OK, I assign
the bug to me (to make sure I'll post it to the core list) and then it
continues as usual with the core list.
