[TYPO3-templavoila] Can't Install Sample Pages During Wizard Set up

Hi List,
The website is up and running and I can access the back end and install
extensions. (Seems permissions are ok)

When I start the Templaviola Wizard, I get step 1 and 2. On step three I
get a page with Step 3: Begin Mapping. The sample pages are not set up and
I cannot see the html elements.
Any direction would be appreciated.
Configuration below.
WebServer: IIS/7.5
Typo3: 4.5.19
Templavoila:1.7
PHP: 5.3.13
MYSQL:5.1.65-community - MySQL Community Server (GPL)
