I must add something else I did in patchset #3. I noticed in the 
definition list that the "Default" item was wrongly aligned when empty. 
I don't know if it's a rendering or a CSS issue. Anyway I thought it was 
better to remove it entirely when not used and added instructions to 
that effect.
