ext:mf_mysql_profiler by Martin Ficzel on forge [1] could help to find 
those sql related bottlenecks: It XCLASSES t3lib_db and logs queries 
together with timing informations to a table (never ever use in 
production ;). A BE module then helps to find slow / very often fired 
queries. #13945 was tackled this way.
