[Typo3-marketing] First Draft: Survey

Hi All.

I have been too busy in the last weeks. Finally i can present you my first
draft of the typo3 survey.

To remind you. I was thinking of creating an survey extension that everyone
who wants to fill it out can send it to a central database.....
