You can create that somewhere we could take a look at, all it has to contain is:

<?php phpinfo(); ?>

and give us a link to it.

From what I see there might be some problem either with your PHP or
the way your Apache supports it. Take a look at your apache Error logs
(both the ones for the website) and the main ones and look for PHP
erros or core dumps there.
