This works:
tt_content.menu.20.1.1.NO {
  doNotLinkIt = 1
  stdWrap.cObject = CASE
  stdWrap.cObject {
   key.field = doktype
   default = TEXT
    default {
      field = nav_title // title
      typolink.parameter.field = uid
      typolink.ATagParams.dataWrap = title="{field:title}"
      stdWrap.htmlSpecialChars = 1
      }

   # 4 = shortcut
   4 = TEXT
   4 {
      field = nav_title  // title
      typolink.parameter.field = shortcut
      typolink.ATagParams.dataWrap = title="{field:title}"
      stdWrap.htmlSpecialChars = 1
      }

   # 3 = external url
   3 = TEXT
   3 {
    field = nav_title // title
    typolink.parameter.field = url
    typolink.extTarget.field = target
    typolink.ATagParams.dataWrap = title="{field:title}"

tt_content.menu.20.1.1.NO.stdWrap.htmlSpecialChars = 0

The other solution, setting:
$TYPO3_CONF_VARS['SYS']['doNotCheckReferer'] = '1'
Works as well

I just had a similar problem today and found this:

Setting DoNotCheckReferer = 1 in localconf / install tool seemed to have done the trick for me (my problem occured only when the used domain had www. before it as opposed to the defined RealURL-BaseURL)

... well I think it worked, it's not completely tested yet.

But about the html-entities: you have "htmlSpecialChars = 1", have you tried setting it to 0?
