you can do different things to get this work.

Switch Perfectlightbox to mootools lib (Contants editor) and manage the
js library with the extension t3mootools.
Check first that both extensions work with the same mootools version.

You can use an +ext template on the site with the form and put the
following in the setup field
page.headerData.1337 >
This will avoid perfectlightbox being included on the proper page
