[TYPO3-english] Session error with captcha form

I have a lot of websites, all with typo3 4.2.8, and a few of them still 
have problems with the captcha error forced by the session fixation 
initial patch, while others use captcha without problems. All sites have 
same software and versions.

Strange thing is this: one installation is a multidomain installation; 
for some websites captcha works, for others don't.

What should I test/debug further in order to check better?
Any hint?
