[TYPO3] th_mailformplus: add tt_address entries AND group

Hey Patric,

As far as  i see, you only need to add data to the tt_adres_mm table 
when you like to add new groups.
So you only need to add data to the 'addressgroup' field if you like to 
assign a tt_adress entry to a tt_adress group
