Did you refresh the table of contents and is it correct? One problem I
had with the doc_template (but I would say it's rather an OpenOffice
bug), it's that the outline level is preserved when you start a new
paragraph after a heading. For example, imagine you've just created a
"Heading 3" title. You press return. The style changes to "Text body",
but the outline level (which you can find in Format > Paragraph >
Outline & Numbering) is still "Level 3". It may be that you have such
"hidden" outline levels which break the rendering.
