Problem: DAM doesn't use Typo3 skin.
This is my final stab at the bug, following comments in the previous 
discussion.
The patch fixes the docheaders, creates a template directory with the 
new templates, and adds markers throughout the extension. It also 
removes some inline styles as well as CSS in the extension and places it 
into an external CSS file. Please note, that not all styles have been 
removed, moved, or changed - that would require too much effort and can 
be aimed for 1.2. This patch only provides a starting point in 
organizing the template, as well as changing the presentation to suit 
Typo3 4.2
