After you insert the image in the RTE, you should be able to edit the
image and remove the title attribute. If not, please report the issue
into the bugtracker: http://forge.typo3.org/projects/typo3cms-core/issues

It is not currently possible to change the default when inserting the
image. Please post a feature request also on TYPO3 Forge:
http://forge.typo3.org/projects/typo3cms-core/issues

Is it possible ? And is it possible to put the
 > image title instead of the image file name ?

The default value of the title attribute should be the title of the
image, not the image file name. If you get the image file name as
default, this is a bug: please report it into the bugtracker.

Best regards,
Stanislas Rolland
