somehow this is a glassbowl problem :-(
I do not encounter any of your issues. So my first question would be:
1) After you upgraded to 4.2.3, did you delete the js files in 
typo3temp/rtehtmlarea?

2) What are your settings for rtehtmlarea in the extMgr?

3) Are you incidentally running DAM. If so, which version, the one in 
TER or the one from SVN (forthcoming 1.1)
