Depends on what you want to do. EXT:piwik is enough to include the tracking 
scripts while avoiding tracking if a backend user is logged in.

You can always add the tracking script by hand of course. This is better 
with recent versions of piwik as the plugin produces "old-style" piwik 
javascript code.

Piwikintegration allows you to be automatically logged into your piwik 
installation and then the piwik admin page is displayed in an iframe in the 
backend.

If you just just a different DB server, then you can adjust the piwik 
configuration file in typo3/config/piwik/piwik/config/... and put the 
address of your piwik server instead of the values from your 
LocalConfiguration.
