[TYPO3-doc] Wiki: Template for Exceptions

Hi Ron, Martin, and Fabien,

starting with 4.6, we have the Exception handler outputting a link to a 
wiki page, like this one:
http://wiki.typo3.org/Exceptionv4/1318283565

Somebody created some nice template structure for the v5 / FLOW3 team. 
Could you try to do the same for v4, too, please?

I just used the template on this page and thus it shows a lot wrong 
information (as it's *not* part of FLOW3 etc.).
