Right! Sorry, if I didn't mention it more clearly.

Correct.

Yes, that's true. DocBook doesn't constrain what XML editor you use. As such 
if the XML editor supports a kind of "author" or WYSYWYG view, you are totally 
dependend on how the editor renders the elements.

The only reliable output is the rendering of your preferred output format. 
This is kind of standardized through the DocBook stylesheets.
