[TYPO3-dam] Exclude fields in DAM

Hi Lorenz,

just tested and gave my +1 on the patch.

What still remains discussable:

I did not set ANY excludefield for my test-user, so now the files are 
not indexed multiple times, but the title field does not get filled -> 
List module shows the files with "[No title]"

Any user that can upload a file should probably be able to set that 
field also, because it is initally filled with the filename that he was 
of course able to set anyway.

Can the check for that field be skipped during indexing?
