[TYPO3-dam] DAM module tx_damindex_index broken

When attempting to access the File/Index page:

.../typo3conf/ext/dam/mod_file/index.php?&SLCMD=&SET=&SET[function]=tx_damindex_index

the DAM module "File" will show only blank page, and the modules  
"file_massupload", "file_list" and "file_upload" also cease to be  
visible.

I recently updated from typo3 4.1.x to 4.2.x - all extension are  
updated and no error messages were displayed.
