[TYPO3-english] (Web) - (Info) - (Site Crawler) will not load

I've installed the Site Crawler extension, but when I go to Web > Info > and select Site Crawler from the drop down list, I get a HTTP 500 error and a blank page. 

I used Fiddler4 to try to capture the request, and here is the URL that it tries to load:
http://servername/typo3/mod.php?M=web_info&moduleToken=3a7a42a19c5891ce4d63959c1a480a580cb50768&id=35&SET[function]=tx_crawler_modfunc1

I'm having the same issue in two different environments.

Environment 1:
Server : Ubuntu 14
Typo3 : 6.2.9
Site Crawler 3.5.0
Bootstrap Package 6.2.8

Environment 2:
Server : Windows
Typo3 : 6.2.6
Site Crawler 3.5.0
Bootstrap Package 6.2.7

Has anyone here see this issue before and know how to resolve it?
