[TYPO3] realurl issue

Hi folks,

We have been working with the realurl extension some time ago.

Many sites were correctly implemented by using this nice extension, however
a couple of months ago we are having serious issues trying to configure this
extension on a typo3 installation. 

First of all, the site mentioned has all requirements to work perfectly:

- We updated the source version from 3.8 to 4.0, and lately to 4.1.5 (we’re
getting the same error with 4.0 or 4.1.5)
- After that, we updated all the tables from the database, ant it’s
appearing to work right
- We reinstall the extensions required like, realurl, and tt_news, without
running into problems
- We add the extra configuration at the vhost.conf files, all the lines
recommended at the realurl’s tutorial were included (actually we used the
code lines from other sites configured and working correctly on the same
server).

Now we’re getting an error like this (enabling SQLDebug in Typo3):

This error refer to the following line:

This error appears
- when I’m logged in as a backend user and try to browse the website at the
frontend (the error appears all the time here)
- when I’m logged out as a backend user, browsing some pages at the frontend
(cannot establish a pattern yet)
- All module pages are working fine

At the bottom of this message, you can find the .htaccess as well as the
vhost.conf lines. We have tried activating one of the files and also both,
without success.
