[TYPO3] Directmail how to configure?

Well, I got that one working quite well using it's own documentation 
(manual.sxw). But I am far from beeing an expert! What is the 
plugin_mgm? I did not have to install something with that name...

I remember that there have been other extensions I needed to install too 
but those were all installed by the extension manager when told to do so.
