[TYPO3] RealURL maps correctly but returns 404 errors

Hi,

I have been struggling with RealURL for some days now and it seems I'm pretty
close. The mapping is working in that it points to http://sitename/feaures.html
but it returns 404 Page not found errors

Typo3 4.01  with pretty much default settings for RealURL as follows. Only non
default is that pages should reflect path names rather than page ids:-
