The install of the extension 'Address list' was not right.

I'm trying to get the extension 'sr_email_subscribe' version 1.1.1 to work
on TYPO3 4.0.2.
I've set up everything as explained in the documentation.

When I test everything and subscribe myself no mail is sent to me and the
mail to the administrator contains the following lines:

Name: ###FIELD_name###
Email: ###FIELD_email###

What can be the problem?
