Another possible starting point might be 'pbsurvey'. It already knows 
about multiple page questionnaires, with conditions on pages, etc. What 
it doesn't do is send an email with the answers in because it stores the 
data in the database. It seems to me that a single hook could be all 
that is needed to allow you to send an email instead of (or as well as) 
storing the data in the database.
