By changes made to t3lib_tceforms.php?

But I know what you mean. The BE doesn't handle required singe-file 
upload fields well.

Besides fixing the bug mentioned above I'd like to see a *real* 
single-file upload field. No min/max-item stuff no misleadg UI (list 
when only one value is valid!).
