[TYPO3-typo3org] Request for TER removal

Hi

Please remove the following extension from TER as they are obsolete,
probably not working and not maintained by us anymore (or let me know
whom to contact directly):

* th_mailformplus_example

Not maintained anymore.

* mailformplusplus

Obsolete since it got renamed to Formhandler.

* direct_mail_123

Obsolete. The feature set is already included in direct_mail by now.

* pagemanager

Not maintained anymore.
