No clue, but I can also confirm that this has happened on some few
installations we host. It is not reproduceable, and happens "once in a
year". We have magic_quotes_sybase=off, so this cannot be the trouble-maker.

Once it happened, I was able to make a snapshot of the "buggy"
temp_CACHED_*.php files. After cleaning the "temp-cache" the site worked
again. But the newly generated files were exactly the same, just a
different timestamp.

Which is why I suspected a race condition with eaccelerator cache.

But since it is not reproduceable, it is somehow difficult to debug.
