How difficult it would be a create context menu item or separate button 
'New Page', which open small dialog with these options
* Page inside (Page as first subpage)
* Page after
* Wizard

Then extra page would be avoided, if the wizard would not needed.
IMO SELECT menu for those option would take too much space.

Concerning the standard page module selecting place would be skipped if 
the page has only one column and there is not existing content elements.

I proposed possibility to add link create new content, when page is 
created - then after saving title would immediately continue creating 
content. Closing page properties and opening content creation is in 
principle unnecessary.

That would avoid some extra click. Endeed I have not experienced them 
annoying.
