I haven't had a thoriugh look on all the available extensions lately, but 
if none fulfils your requirement there are only the options of 
coordinatened extension of an exteing one, hacking an existing one or 
writing one.

I'd say no, to your question if TYPO3 really needs one. OTOH, many 
galleries are old and it might be easier and more maintanable if you start 
a new one from scratch.

An interesting feature of a new gallery could be to work with DAM. Maybe 
it's better to have more than one extension to cope with "native"/DAM image 
sources, eg "gallery", "gallery_native", "gallery_dam".

And of cource it could support the mythical general category tree :-)

PS: If you need l10n I can send you my yet inpulished verision 0.10 of 
gsi_slideshow. It features full translation of all fields (which incl. meta 
data like location, photographer, artist).
