[TYPO3-core] RFC #16939: ExtJS Tooltip / CSH styling

> 2) The cursor change to a question mark isn't working on FF and Chrome.

Have no clue, think it's not related to this.

> 4) The tooltip title isn't showing on IE8. See screenshots.
Fixed, line-height and font-size was required.

I also changed the following things
* unset ExtJS shadows, use CSS3 shadows
* add a label "Loading help.." for the loading-time this fixes
   - weird-looking tooltip during first load
   - moving from one label to the other, shows the old one for a moment 
(until new text is loaded)
* anchor now points to the vertical center of the label / ? icon, not to 
the bottom
