Problem:
htmlArea RTE toolbar and context menu styling needs adjustments.

Solution:
The attached patch fixes various small styling issues such as 
positioning of icons on toolbar buttons and on context menu entries, 
size of toolbar buttons in Opera, flickers in the context menu, height 
of toolbar buttons compared to height of combo boxes, etc.
