I've uploaded testcase test.t3d here http://bugs.typo3.org/view.php?id=13901

How to reproduce:

1. Render the page
2. Check filename of the image on the page
3. Clear cache
4. Reload page
5. Check the filename again (will be different)
6. Check typo3temp/GB/ - you will find there 2 files with same sizes 
have been just created
