[TYPO3-core] RFC #1104: Setting file fields in TCA to "required" makes it impossible to save the record

Problem:
Setting file fields in TCA to "required" makes it impossible to save the
record. (Or rather, setting minitems > 1, which is like required.)

Solution:
Add the newly uploaded element to the count of total elements. This also
makes maxitems work correctly for newly uploaded items.
