[TYPO3-core] RFC #11813: Code cleanup: refactoring t3lib_db to match CGLs

FYI: Committed to trunk rev. 6203

To make sure this doesn,t break anything, committed, attached 03 patch
is a pure cleanup without any functional changes.

For easy post-commit reviews find attached second version with stripped
whitespace changes.
