Problem:
The new "Refresh-Login" account is not working properly. Even if the
correct password is entered a dialog appears with "Password incorrect".

Solution:
The challenge hash in the form does not match with that one stored on
the server. So, the challenge hash is created just before the form gets
send and used to wrap the password.

Notes:
The relogin only works with challenged and superchallenged, but not with
OpenId or SaltedPasswords.
