[TYPO3-core] FYI: icons fixed and changed

Hi all,

there was a small one pixel error in the refresh icon found on top of 
the file and page tree (probably used elsewhere, too), this has been 
fixed in 4.2 and trunk.

Additionally the icons for new file and file upload in the file module 
have been replaced with new ones that are more consistent with the 
existing ones. The new icons are checked and approved by Jens, Benni 
also checked them. Please find the new icons attached.
