[TYPO3-core] FYI: backend.php now prepared for access check on toolbar items, interface changed !

Hi all,

as a late follow up to the cleaner backend project I just committed a 
change that enables access checking for toolbar items (yeah, I knew I 
forgot something... sorry for that)

attention Benni, Steffen, and Dmitry: the interface for the toolbar 
items has changed, as you already have extensions using that interface 
you need to adopt the changes (except for Benni) - sorry for the 
inconvenience!

Note: the real access check for the existing toolbar items still needs 
to get implemented, I know that Steffen was already working on that by 
just copying the code from the old equivalents. The according places are 
marked with "// FIXME - needs proper access check".

Another note: another minor change slipped in with this commit as I 
forgot to remove it before committing. However this is not dramatic at 
all as I only removed some legacy JS code in backend.php - some left 
over from alt_main.php which couldn't work with backend.php anyways.
