[TYPO3-core] RFC: #8203: Broken icons when creating +ext template

Hey guys,

This is a SVN patch request.

Branches: 4.2 only

Problem:
When creating a +ext template, the warning icons next to each button are 
broken. The icon HTML has a duplicated src tag and the file path is 
slightly off.



Solution:
Adjust the HTML and the file path.

Notes:
Before and after screenshots at the bugtracker. I'm leaving the <BR> 
tags alone for the time being, since they're sprinkled through the module.
