[TYPO3-core] Gremlin #1625: Create an icon for the "not in menu" pages

User writes:

Pages with the page not in menu flag use the same icon. This aanoys me a
liottle as I cannot easily see the state of a page.
Suggestions include a small crossed out M over the normal page icon.


Solution:

- Change icon for "Hide in menu" by forcing a different doktype if nav_hide
  is set in the pages row


Notes:

Additionally, I removed "Not in menu" from the list of available doktypes.
It is NOT removed completely, it just can't be selected anymore.
